Emergency Care Info

We do NOT provide Emergency/Crisis Care - Please Call 911 or call/Text 988 Suicide and Crisis Lifeline or Seek care/help at the nearest Hospital/Emergency Department, and follow your Emergency/Crisis Plan.

Payment Details

Our goal is to help you achieve optimal mental health and wellness, and we are committed to providing the highest quality care. However, payments and balances should be cleared within a couple days of our services. We do require all our clients to maintain an active credit card on file for deductibles, copays and balances. 

Attendance Policy

Our providers make themselves available for your appointment and their time is blocked for you. We do not double book our providers and in order to ensure value of their time we do charge no show fees. No show/late cancellation fees are applied  to all cancellations with less than 48 hours notice. 

Reschedule Policy

After a missed appointment, clients are responsible to reach out to clinic for refills in a timely manner prior to running out of refills. Clients are required to schedule a follow up appointment to be eligible to obtain refills on their scripts. 

Legal Paperwork Policy

These services are only available to our established patient(s) as we know them, know their history, know their Mental Health condition(s), treatment plans and we can confidently complete these types of paperwork. An establish patient is one that has been a patient with our practice for more than 6 months, or has had at least 4 or more appointments. Patient is responsible for the charges related to this service. Please see Fee Section.

 NOTE*  Please be advised that Theory Of Everything does not engage in or offer services related to mental health evaluation or treatment concerning legal matters, court proceedings, or legal representation. We do not have the necessary resources to facilitate such services. We strongly advise seeking an in-person provider who specializes in these specific areas for appropriate evaluation and treatment. 

Paperwork Policy

Our practice is committed to providing high‑quality clinical care, treatment planning, and documentation directly related to patient health. However, we do not complete or submit disability applications, disability determination forms, FMLA paperwork, or any related administrative documents on behalf of patients.This includes, but is not limited to:

  • Short‑term or long‑term disability forms
  • FMLA certification forms
  • Work‑leave or job‑accommodation forms
  • Social Security disability paperwork
  • Insurance‑based disability evaluations

Rationale

 

These forms often require specialized evaluations outside the scope of our practice.

Disability and FMLA determinations are legal and administrative processes requiring documentation from providers specifically trained or designated to perform such assessments.

Maintaining this practice ensures that our clinical time is dedicated to direct patient care and treatment.

What We Can Provide:

 While we do not complete disability or FMLA paperwork, we may provide:

  • A general treatment summary upon patient request
  • Verification of attendance for appointments
  • A brief statement confirming diagnosis and treatment plan (when clinically appropriate)

These documents do not include functional capacity assessments, work‑ability determinations, or recommendations for leave. 

What can Patients do:

Patients requiring disability evaluations, FMLA certification, or work‑leave documentation should seek assistance from:

  • Their primary care provider
  • A specialist trained in disability evaluations
  • An occupational medicine provider
  • A designated FMLA/disability evaluation service
